substance users within a supportive service which can
respond to their identified needs in an holistic way,
focusing not just on their substance use and showing
individuals continued respect and having a commitment to
harm reduction.
Triage workers will support progress with the
individual via a range of engagement techniques
including access to a holistic assessment of their
needs. Individuals will then be supported to look at
options available and what they can expect from these.
This will be done by a care coordination role and will
be actioned by a care plan written in partnership with
the individual.
